The Vascular Coder offers vascular operative report auditing to help identify any coding and/or documentation deficiencies of previously coded op-notes. We will code each report and write a synopsis of our findings identifying any coding discrepancies and detailing any action to be taken.  Also included in the synopsis will be documentation deficiencies (if any) and the codes that could have been reported or should not have been reported due to the lack of documentation.

We can perform audits of any size that our clients might require and work closely with each client to determine the appropriate number and types of charts that need to be audited. A majority of audits are done in a staged format where we audit a small number of surgical reports in order to determined the problem areas and use those findings to develop a more focused audit.
